A rebate check will be mailed to purchaser listed on this form. Email addresses will only be used to notify you of your rebate status.

All rebate forms must be received within 45 days of the purchase date. A separate rebate must be completed for each heat pump water heater purchased. Please allow 4-6 weeks for processing. This offer is available through Dec. 31, 2024 or while funds last.
